A shocking bungee jumping incident in Brazil has sparked outrage and renewed concerns about safety standards in extreme adventure sports after a 21-year-old woman died during a supervised jump.

Maria Eduarda Rodrigues de Freitas was taking part in a rope-jump activity on the abandoned Ponte do Esqueleto bridge in Limeira, São Paulo state, on Saturday, June 13, 2026.

According to reports from the São Paulo Public Security Secretariat and local military police, the young woman was lifted into a “Superman” pose by three instructors before being pushed off the 40-meter-high structure.

However, investigators allege that the safety cord was not attached at the time of the jump. She fell to the ground and died instantly from multiple traumatic injuries.

Following the incident, six people connected to the organising companies — Entre Cordas and Ih Voei — were arrested.

The three instructors who physically carried out the jump have been identified as Luis Felipe Feliciano Egoroff, Vitor de Freitas Gonçalves, and Maicon Fernandes Cintra.

They have been charged with homicide with eventual intent, according to local authorities.

The incident has sent shockwaves online, with many questioning how such a basic safety failure could happen during a supervised extreme sport activity.

Authorities are continuing investigations into the companies involved and whether safety protocols were followed before the fatal jump.

The case has also reignited wider concerns about regulation and oversight in adventure tourism, where high-risk activities often depend heavily on strict procedural checks.

What was meant to be an adrenaline-filled experience has now turned into a devastating tragedy — and a stark reminder of what can go wrong when safety steps are allegedly missed.
